Supplemental Life Insurance
Some employees may elect to purchase Supplemental Life Insurance coverage. If eligible, you may purchase coverage for yourself. And for Managers, if you purchase this coverage, you may elect to purchase life coverage for your spouse/domestic partner and eligible dependent children. Employees pay the full cost for this coverage.

Employee Supplemental Life
If eligible, you may purchase Supplemental Life Insurance coverage in $10,000 increments, up to a maximum benefit of $300,000. The coverage you purchase cannot exceed three times your base salary. All Managers, All Unions can purchase up to five times your base salary to a maximum of 1,000,000.
Premiums are based on your age and the coverage level you select. You pay the full cost for this coverage. Refer to Plan Costs for the current premiums.
You must designate a beneficiary(ies) for the Supplemental Life Insurance benefit. (A beneficiary is the person who receives the benefit in the event of your death.) You may add or change a beneficiary by completing the Beneficiary Designation Change Form and returning it to the Employee Benefits Center (EBC). You may make changes to your beneficiary designation at any time.
Spouse/Domestic Partner Life Insurance
If you purchase Supplemental Life Insurance coverage for yourself, you may purchase coverage in $10,000 increments for your eligible spouse/domestic partner. You may elect up to $150,000 in coverage; however the coverage you purchase for your spouse/domestic partner cannot exceed your Supplemental Life coverage amount.
Premiums are based on your spouse's or domestic partner's age and the coverage level you select. You pay the full cost for this coverage. Refer to Plan Costs for the current premiums.
You are automatically the beneficiary of this coverage.
Dependent Child Life Insurance
If you elect Supplemental Life Insurance coverage for yourself, you may elect to purchase dependent child life insurance for your eligible dependent children in one of the amounts below:
- $2,500
- $5,000
- $7,500
- $10,000
Premium costs vary only by the option you choose - you pay the premium, regardless of the number of children you choose to cover. You pay the full cost for this coverage. Refer to Plan Costs for the current premiums.
You are automatically designated as the beneficiary for this benefit.
Supplemental Life Insurance Costs
You pay the premiums for Supplemental Life Insurance coverage. Costs for employee and spouse/domestic partner life insurance are based on the covered individual's age and coverage level chosen. Costs for dependent child coverage are based on the coverage level you select only.
